Who Dyad is for
Families where one person is living with dementia or serious memory loss, and another family member is the main caregiver. To join, the person with dementia usually needs to have both Medicare and Medicaid, and be enrolled in a health plan that partners with us.
If you aren't sure what insurance they have, join the waitlist anyway — we can help figure it out.
What Dyad does
We pair every family with a care navigator who learns your family and checks in regularly — in your language, in your community. When you need extra dementia-specific guidance, our dementia specialist is available within days, not months — working alongside your existing doctors, not replacing them. When the caregiver needs a break, respite care is scheduled automatically — often before the caregiver has to ask.
Everything is delivered through your health plan and the doctors you already see. Nothing replaces them; we add what's missing.
